New to Lakeside

Welcome to Lakeside!

In order to enroll you at Lakeside, you will need to complete and submit the following forms and information.

• New Student Registration Form

• Home Language Survey

• Request for Cumulative Folder

• TB Risk Assessment for School Entry 

(NOTE: The TB Assessment must be signed by the student’s doctor.)

• Proof of Residency : 

  • 1 item from the following list dated within 60 days of enrollment is required:

-Title or escrow papers, mortgage statement, property tax payment receipt, or homeowner’s fee statement
-Lease agreement/rental contract with landlord’s name, address, and phone number or current rent receipt
-Utility service contract, statement, or payment receipt—gas, electric, water/sewer, cable, trash (telephone/cellular phone bills are not accepted)

• Copy of Birth Certificate

• Immunization Record

 

You may download these PDF’s, fill them out and email them back to ljsd@lakesidelosgatos.org.  Or you may contact the district office to have these forms mailed to you. And of course, you may always stop by the office and pick up the forms.​​

 

Once you have returned these form you will be given information on how to complete your online registration.
